Disease Management Programs Inventory

About the Program

CQC has worked with California health plans and physician organizations to develop the Disease Management Programs Inventory to help groups and plans better meet their patients’ disease management needs.

Over the years, more programs have become available to help patients manage chronic conditions. These programs have been traditionally sponsored by health plans, however, physician organizations experience a growing need to either access existing resources, or coordinate resources as they build their own capabilities.

The Disease Management Programs Inventory is a Web-based resource intended for California physician organizations and plans. It contains current and standardized information on disease management programs offered by six California health plans.

Anyone from a California health plan or physician organization can register to use the Inventory.
