Clinical Care Collaborative

Overview

The Clinical Care Collaborative is a program designed to measurably improve care for a defined population of patients.  Track 1 (Building an Effective Registry) is a 3-month program for physician organizations without an existing registry reporting tool.  Track 2 (Optimizing Registry Use to Improve Patient Care) is a 12-month program for organizations with an effective registry, including those that complete Track 1.

The next program will be held in 2010 - dates TBD.
