Participation Requirements

• This collaborative is designed to encourage teams and individuals to share experiences with each other and make learning available to every participating group.
• Participants must be able to attend sessions and events as outlined in the Schedule.
• Attendees are clinical teams from physician organizations, including medical directors, quality improvement staff, case managers, and disease managers.

Cost

All Chronic Disease Learning Exchanges are free, but space is limited.
