About the Program

Overview

CQC Chronic Disease Learning Exchanges cover a variety of topics such as diabetes, depression, and complex case management.  Topics are proposed by physician organizations that are part of the CQC Regional Learning Networks, the CQC Steering Committee, and attendees of the learning exchanges.  Meetings focus on what is required to make improvements in chronic care, how physician organizations can make and measure improvements, and practical tools to get started right away.

Approach

Learning Exchanges are quarterly half-day or full-day onsite sessions located in the Los Angeles, Orange County and Inland Empire regions.  Onsite exchanges may be followed by one or more teleconferences to reinforce learning or address other aspects of the topic.

The meeting structure allows for substantial interaction between attendees and presenters/facilitators. Meetings often include open forums to share case studies, challenges, and successes with improving care.  Faculty include local experts such as Dr. Stuart Levine, Corporate Medical Director, of Healthcare Partners and Dr. Neil Solomon, VP & Senior Medical Director of Quality & Medical Policy, of Health Net.  Peer presentations are given by California physician organizations with practical experience in the selected topic.

Attendees are clinical teams from physician organizations, including medical directors, quality improvement staff, case managers, and disease managers.
