Chronic Disease Learning Exchange

Overview

CQC Chronic Care Disease Learning Exchange provide physician organizations an opportunity to learn from experts and peers about practical strategies to effectively manage chronically ill patients.  Learning exchanges cover various topics such as diabetes, depression, and complex case management to name a few.  Learning exchanges are typically held quarterly in Southern California for a half to one day of onsite learning.  Attendees are clinical teams from physician organizations including medical directors, quality improvement staff, case managers and disease managers.
