California Quality Collaborative (CQC) is a healthcare improvement organization dedicated to advancing the quality and efficiency of patient care in California.

CQC's expert quality improvement programs transform healthcare delivery across physician groups through collaboration. The organization is supported by a state-wide leadership alliance of healthcare purchasers, providers, and health plans, all working toward a shared goal of accelerating quality improvement.

CQC Program Areas

The Improving Patient Experience Collaborative is a year-long program aimed at improving an organization's PAS Score in 18 months.  This collaborative will help to prepare organizations in the implementation of key changes at practice sites to improve and sustain patient ratings.

The Efficiency Collaborative is a year-long program aimed at providing physician groups with the right tools to successfully reduce the overuse and misuse of clinical services in five clinical areas.  Participants will learn from expert faculty and each other 1) effective processes for identifying and targeting unnecessary practice variation in select measures, 2) which interventions work in these measures, and 3) how to effectively engage physicians to make improvements in these measures.

Improving Clinical Care  2008 info coming soon

During this year-long collaborative, physician group teams will work together to implement key changes known to improve chronic care.  Teams will learn how to convert data into actionable reports and how to set up systematic population management programs that will make a difference for a population in patients and improve P4P clinical scores.

Building Capacity for Change

The Building Capacity workshops are aimed at building capacity within an organization to successfully facilitate change.  Learn the practical tools and techniques that will enable an organization or an individual practitioner to deliver improvements in complex clinical settings.

The Disease Management Project is aimed at improving coordination between plans and physician groups around disease management programs for patients they share.  The multi-stakeholder task force behind this project developed an online tool to improve coordination of disease management efforts.

Regional Learning Networks

Learning networks, in the Inland Empire and starting soon in LA/Orange County, aim to build relationships and partnerships in a region to accelerate improvement in clinical care and patient satisfaction.  Physician groups, health plans, hospitals, and local medical associations come together to share best practices, resources, and exchange lessons learned in quarterly meetings, teleconferences and via an electronic newsletter.

Chronic Disease Learning Exchanges

CQC is hosting a series of learning exchanges on various topics in chronic disease management.  These learning exchanges will take place by teleconference and half-day on-site sessions both in Northern and Southern California.  Topics include: COPD, Depression, Cardiovascular Disease, CHF, Asthma, and more.
